AI in Nursing Documentation and Charting Survey: Practices, Oversight, and Impact

Goal: This survey aims to better understand how artificial intelligence (AI) tools are being used in nurses’ clinical documentation and charting across healthcare settings. We seek to capture nurses’ experiences, organizational practices, and observed impacts on workflow, decision-making, and patient care.


Participating in this survey supports and advances the work of our 2026 Advocacy Institute Fellows and AI + Documentation and Charting Campaign.


Your Impact: Your insights, perspectives, and experiences will help inform our Advocacy Institute Fellows and strengthen our policy recommendations. Your input will help identify areas where guidance and oversight are needed, and it will support the responsible and effective use of AI in nursing practice. Ultimately, this work will help shape AI policies that reduce documentation burden and restore time for care.

Standardized Definitions:

Documentation and Charting: The process of recording patient care, clinical assessments, interventions, and outcomes within the medical record, including both structured data entry and narrative clinical notes. For the purpose of this survey, documentation and charting are used interchangeably to refer to this process.


Artificial intelligence (AI) tools: Software or systems that use data-driven algorithms to assist with tasks such as generating, summarizing, or supporting clinical documentation, decision-making, or workflow processes.
